OUTREACH SERVICES

Women frequently face significant barriers to engaging in treatment and may have difficulty balancing the demands of motherhood and interpersonal relations with the tasks required to participate in a comprehensive intervention. Women referred to treatment by the Department of Human Services and the Department of Children and Family Services may be eligible for outreach services from experienced community Case Coordinators. Case Coordinators can provide the following services:

  • Coordination of screening and referral between DCFS, DHS, and Duane Dean Behavioral Health Center
  • Provide client-focused case coordination that addresses barriers to engaging in treatment
  • Transport clients to treatment and related appointments
  • Complete home visits and follow-up contacts with women who fail to enter treatment, or who drop out of treatment
  • Link clients to community services
